Just wanted to post a simply play by play here to help everyone out...
Before each visit, on the day of the visit, please fill out the ATD and the Medical history forms. (For those with lots of medications, bring us a list we can scan in to make this easier). The ATD from must be filled out the same day.
Text (preferable) or call from you car to let us know when you have arrived. We will let you know when we are ready for you.
Unless a guardian is needed, everyone should come alone- we can't have people in the waiting room at this time.
Help us conserve PPE-Wear a mask. If you don't have one, wait outside the door and we will bring one to you.
You will fully be cared for in the treatment room to minimize exposures. If you need any clarifications on your account from the front desk, please call ahead and speak to them.
